A Custom Form is a form schools build in EnrolHQ to collect additional information from families. Common examples include consent forms, medical updates, and interview documents.

Custom Forms connect to EnrolHQ's standard workflow tools to improve school productivity and compliance. Build a form, add as many questions as required (with their own answer types), include explanatory text, add sections and even split the form into multiple steps. Schools use Custom Forms for things like:

• Media consent forms
• Medical consent forms for administering medication
• IT agreement forms
• Updating medical details and emergency contacts
• Updating learning support
• Additional document uploads
• Co-curricular sign-ups
• Bus and travel arrangement forms
• Payment of fees in advance

Parents access Custom Forms through the Enrolment Dashboard. Each form shows a flow indicator in the side menu and carries the school's branding.

Many schools prefer to make Custom Forms mandatory prior to accepting an offer so all paperwork is complete before the offer is accepted and payment is made.
